What does an Independent Care Needs Assessment involve?

Your assessment will be carried out in person (face-to-face) by an experienced health or social care professional.

During the assessment, we will take time to understand:

Your health, wellbeing, and daily living needs

What you can manage independently and where you may need support

How you would like to be supported now and in the future

Your current living arrangements

Your cultural, religious, and personal preferences

Information from your carer, family member or representative (if you would like them involved)

What happens after the assessment?

Within 7 days of your assessment, you will receive a detailed written report, including:

  • A clear summary of your assessed needs

  • Professional recommendations for appropriate support

  • Guidance on next steps and available options

If you need care, UB Healthcare offers free support and advice to help you find the right care. This may include:

  • Helping you explore home care or residential care options

  • Signposting to NHS services, social services, voluntary and community organisations

  • Connecting you with trusted local support and providers

Our aim is to ensure you feel informed, supported, and confident in your decisions.

With your consent, the assessor may also speak with other professionals involved in your care, such as your GP or nurse, to ensure a complete and accurate understanding of your needs.

The assessment considers both your current needs and how your needs may change over time, helping you plan with confidence.

Our independent assessment does not replace the statutory care needs assessment provided by your local authority

You (or someone acting on your behalf with your permission) can still request a local authority assessment at any time If you lack the mental capacity to decide, your appointed representative can request an assessment in your best interests If you are a carer, you are also entitled to request a carer’s assessment through your local authority.

UB Healthcare does not carry out financial assessments (means tests). Your local authority is responsible for assessing whether you are entitled to financial support and what contribution you may need to make towards care costs.

Eligibility for CHC is determined solely by the NHS through a formal statutory process, which may begin with a CHC Checklist carried out by NHS professionals.

The CHC Checklist is a screening tool used by NHS professionals to help decide whether a full CHC eligibility assessment should be carried out.

While UB Healthcare cannot determine eligibility or carry out statutory CHC or local authority assessments, our independent care needs assessment can help by:
