Meet our Care Advice & Placement team

  • With over 25 years in the care sector, my journey began with a family health issue that sparked my passion for caregiving. Initially rooted in customer service and management, I developed skills in leadership, communication, and problem-solving, which proved invaluable in my transition to care roles.

After a few years as a carer, I moved into hospital discharge and patient flow management, supporting various NHS pathways across the UK. My combined skills facilitated rapid progression to managing community placements, overseeing End of Life care, and piloting innovative schemes.

My interest in mental health and complex care led me to coordinate care packages for children with complex needs, a rewarding experience working closely with families. I then managed independent living facilities for adults with mental health conditions, including psychosis and severe autism, deepening my passion for the field.

Following my father's dementia diagnosis, I became a CQC registered manager of a residential dementia care home, expanding my skills in dementia care.

Currently, at UB Healthcare, I leverage my experience to support the elderly, coordinating complex mental health and physical cases.

My career blends customer service, management, and care, enabling me to significantly impact the lives of those I support.

  • I have been working in the health and social care field since 2007, starting out as a support worker in a school for autistic children. Over time, I progressed to the role of care home manager and obtained the level 5 QCF health and social care—leadership and management certification. In addition, I served as a trainer for safeguarding, mental capacity, and first aid.

In 2016, I transitioned to working in hospital discharge, where I assisted patients and social workers in arranging care solutions. In 2017, I was tasked with initiating a self-funding placement scheme in Hampshire, a responsibility I have held ever since, helping 100s of individuals to find the right care. This position has been incredibly fulfilling and has provided me with great satisfaction in my career.

Throughout my time in this role, I have had the privilege of assisting numerous families in finding suitable care for their loved ones. I am dedicated to providing a comprehensive and compassionate service to our clients. I understand the challenges families face when seeking care, and I am committed to alleviating their stress and providing unwavering support until their loved ones receive the care they require.

Testimonials

My father recently went into hospital. He is 99 and was not well. But he pulled through. The hospital gave me UBH Healthcare’s telephone number to talk to - to arrange ongoing healthcare.

I spoke to Tracey about the situation and right from the start you could tell that she was empathic and supportive of my situation with my father. She told me exactly what she was going to do - and did it!

The result was that 6 days my father had been discharged from hospital and arrived at his new nursing home. I was kept informed of everything on a daily basis.

So, my review is not only for UBH Healthcare but also, specifically, for Tracey. Not only did she do everything she promised, but she also put me at ease (and made me laugh) despite the urgency of the situation.

It's the first time in many years that I have seen him laugh and smile. The managers and care team are very friendly and supportive. .'
