These assessments are usually conducted by local authority social care teams to determine what support and services may be required, such as at-home support, equipment, or residential care.

UB Healthcare offers a personalised, independent care needs assessment for individuals who are self-funding or who would like an independent, professional opinion to better understand their current and future care needs.

What does an Independent Care Needs Assessment involve?

Your assessment will be carried out in person (face-to-face) by an experienced health or social care professional.

During the assessment, we will take time to understand:

Your health, wellbeing, and daily living needs

What you can manage independently and where you may need support

How you would like to be supported now and in the future

Your current living arrangements

Your cultural, religious, and personal preferences

Information from your carer, family member or representative (if you would like them involved)

What happens after the assessment?

Within 14 days of your assessment, you will receive a detailed written report, including:

  • A clear summary of your assessed needs

  • Professional recommendations for appropriate support

  • Guidance on next steps and available options

If you need care, UB Healthcare offers free support and advice to help you find the right care. This may include:

  • Helping you explore home care or residential care options

  • Signposting to NHS services, social services, voluntary and community organisations

  • Connecting you with trusted local support and providers

The cost of UB Healthcare’s in-person, personalised Independent Care Needs Assessment is: £750

Booking your assessment is simple:

  1. Book a free pre-assessment consultation via our website

  2. Your dedicated care coordinator will contact you

  3. If appropriate, an in-person assessment will be arranged at a time that suits you

  4. You’ll receive your assessment report and recommendations within 7 days

With your consent, the assessor may also speak with other professionals involved in your care, such as your GP or nurse, to ensure a complete and accurate understanding of your needs.

The assessment considers both your current needs and how your needs may change over time, helping you plan with confidence.

Important information

  • Our independent assessment does not replace the statutory care needs assessment provided by your local authority

  • You (or someone acting on your behalf with your permission) can still request a local authority assessment at any time

  • If you lack the mental capacity to decide, your appointed representative can request an assessment in your best interests

  • If you are a carer, you are also entitled to request a carer’s assessment through your local authority

UB Healthcare does not carry out financial assessments (means tests). Your local authority is responsible for assessing whether you are entitled to financial support and what contribution you may need to make towards care costs.
